In Ayurveda, Shiro refers to the head, and Basti means to retain or hold. Shiro Basti is an advanced, profound neurological treatment where a specialized cylindrical cap is fixed to the head utilizing a leak-proof dough seal.
Once sealed, the cap is filled with significant volumes of highly concentrated, warm medicated oil. The scalp is literally completely submerged in an unbroken pool of medicine for up to an hour.
The head is the master control center (Murdha) governing all sensory and motor functions. When Vata dosha severely dries out the cranial nerves—causing conditions like facial paralysis, severe memory loss, unyielding insomnia, and optical degeneration—simple oil massages are insufficient. Shiro Basti forcefully permeates the scalp barrier, driving heavy lipids directly into the cranial tissue, hydrating the nervous system and profoundly regenerating damaged neural pathways.

Core Therapeutic Advantages
- Treats Facial Paralysis: Highly potent in the recovery of Bell’s Palsy, facial numbness, and trigeminal neuralgia by restoring cranial nerve conduction.
- Alleviates Cluster Headaches: Deeply nourishes dried-out cranial tissues, vastly reducing the frequency and intensity of severe cluster and tension migraines.
- Cures Severe Sleep Disorders: Extinguishes chaotic, racing thoughts by physically weighing down the Vata dosha, forcing the brain into deep, regenerative sleep.
- Improves Memory Loss: Regenerates cerebral circulation, halting cognitive decline and improving focus, memory retention, and mental clarity.
- Arrests Hair Fall: Submerging the hair follicles in potent medicated oils profoundly strengthens the roots, preventing premature baldness and greying.
- Soothes Optic Nerves: While applied to the head, the cooling and nourishing effects dramatically reduce severe optical burning and eye strain.
Phased Treatment Procedure
Phase 1: Purvakarma
The physician evaluates the patient’s scalp for any severe fungal infections, open wounds, or extreme Kapha disorders (like severe chest congestion), where heavy oil pooling on the head might be contraindicated.
For optimal oil contact, shaving the head is sometimes clinically recommended (though not always mandatory). If kept, hair is tightly bound to allow the dough ring to seat firmly against the scalp.
The patient sits upright. A thick strip of leak-proof dough (black gram flour) is pasted around the circumference of the head. A cylindrical leather or waterproof cap (Yantra) is then firmly affixed to the dough.
Phase 2: Pradhana Karma
Specific highly-medicated, neuron-nourishing oils (like Ksheerabala 101 or Dhanwantharam Tailam) are heated to a strictly calibrated, deeply comforting lukewarm temperature.
The warm oil is precisely poured into the cylindrical cap until it rises about an inch above the scalp. The head is completely submerged in the standing pool of warm medicine.
The patient sits totally still in profound silence. The oil is retained for 30 to 50 minutes. The therapist routinely checks the temperature, replacing cooled oil with fresh, warm oil to maintain continuous heat.
Phase 3: Pashchatkarma
Once the retention time is complete, a small gap is created in the dough seal, and the oil is safely drained out. The cap and dough are then removed.
The head and neck are vigorously wiped, followed by a light scalp and shoulder massage to dissipate the remaining heat and push the absorbed oils deeper into the nervous pathways.
A warming herbal powder (Rasnasnadi) is rubbed into the crown to prevent catching a cold. The patient must take a warm bath and strictly avoid cold winds and physical exertion.
