In Ayurveda, Janu translates to the knee joint, and Basti means to retain. Janu Basti is a highly specialized therapy where a reservoir of warm, customized medicinal oil is retained over the knee joint to deeply lubricate and repair the complex joint capsule.
The knee is a major Sandhi (joint) prone to high wear and tear. Ageing and excess Vata dosha cause the synovial fluid (Shleshaka Kapha) to dry up, leading to painful bone-on-bone friction known as Sandhigata Vata (Osteoarthritis).
Janu Basti is uniquely capable of combating this degeneration. By pooling heavy, nourishing herbal oils directly over the patella and applying continuous heat, the active lipid-soluble herbal compounds bypass the gastric route and penetrate directly through the skin into the joint space. This triggers immediate pain relief and long-term structural lubrication.

Core Therapeutic Advantages
- Halts Osteoarthritis: Considered the premier non-surgical treatment to halt the progression of knee osteoarthritis and cartilage degeneration.
- Reduces Friction Pain: Significantly lowers knee joint pain and grating sensations (crepitus) occurring during movement or climbing stairs.
- Deep Cartilage Nutrition: The targeted heat and Snigdha (unctuous) oil penetrate deep to nourish drying knee cartilage and local ligaments.
- Increases Synovial Fluid: Helps stimulate the natural production of synovial fluid, acting as a lubricant generator for dry, stiff joints.
- Improves Flexion: Relaxes tight hamstrings and quadriceps attached to the knee, dramatically improving the range of motion and joint flexion.
- Strengthens Patellar Tendons: Rebuilds tensile strength in the patellar tendon and surrounding supportive structures after sports injuries.
Phased Treatment Procedure
Phase 1: Purvakarma
Clinical evaluation of the knee joints to grade osteoarthritis, check for swelling (effusion), and assess crepitus (grating sounds during movement).
Gentle application of medicated oil around the knee joint and patella to stimulate blood flow and relax the surrounding ligaments and tendons.
A mild pulse of herbal steam is directed at the knee to soften the tissues and open local channels.
Phase 2: Pradhana Karma
The patient lies on their back. A thick, circular retaining wall made of black gram (Urad dal) dough is built precisely over the knee joint, centering the patella.
Customized warm Ayurvedic oil (like Murivenna, Ksheerabala, or Dhanwantaram) is gently poured into the dough ring until the knee is fully submerged.
The oil is kept consistently warm for 30 to 45 minutes by continuously swapping cooled oil with freshly heated oil.
Phase 3: Pashchatkarma
The therapeutic oil is sponged out of the reservoir, and the urad dal dough ring is gently removed and discarded.
A very gentle massage is performed over the patella and surrounding joint capsule to promote deep penetration of the herbal extracts.
The patient is advised to limit excessive walking, running, or stair climbing immediately post-treatment to allow the joint capsule to heal.
