Thalam (sometimes called Shiro Lepam) is a unique, highly specialized outpatient Ayurvedic therapy. It involves creating a thick, intensely cooling medicinal paste from raw herbal powders mixed with medicated oils or fluids, which is then retained directly on the vertex (crown) of the head.
It serves as a rapid, targeted intervention when full-body therapies or extensive oil pouring (like Shirodhara) are either unnecessary or contraindicated, yet the brain desperately requires immediate pacification.
In cases of extreme stress, anger, or nervous exhaustion, Pitta (heat) and Vata (erratic wind) violently rush upward, literally "overheating" the brain. Thalam acts as a profound, localized deep-freeze. The thick wet paste pulls the trapped vascular heat out of the cranial cavity, instantly lowering blood pressure, ceasing throbbing migraines, and coercing the frantic nervous system into an immediate state of heavy, tranquil sleep.

Core Therapeutic Advantages
- Cures Insomnia: Acts as a profoundly heavy, cooling weight on the crown, switching off racing thoughts and instantly inducing deep, restorative sleep.
- Reduces Blood Pressure: Highly effective as an immediate intervention to physically cool the brain and significantly lower acute stress-induced hypertension.
- Relieves Migraines: Immediately draws heat and congested vascular pressure away from the frontal lobe, curing throbbing migraines and tension headaches.
- Calms Severe Anxiety: Acts as a direct pacifier for the aggravated Vata and Pitta doshas in the mind, effectively treating panic attacks and chronic nervousness.
- Halts Hair Fall: The deep cooling effect prevents the "burning" of hair follicles caused by extreme mental stress, halting premature greying and hair loss.
- Soothes the Eyes: The cooling action reflexologically travels down the optic nerves, drastically reducing burning sensations, redness, and strain in the eyes.
Phased Treatment Procedure
Phase 1: Purvakarma
The physician evaluates the patient for acute mental distress, severe heat (Pitta) in the brain, anxiety disorders, or extremely high blood pressure that requires immediate cooling.
Specific cooling and brain-calming herbal powders (like Amalaki, Brahmi, or Sandalwood) are selected based on the exact nature of the neurological imbalance.
The dry herbal powders are thoroughly mixed with a potent liquid medium—such as medicated oils (like Ksheerabala), rose water, or medicated buttermilk—to create a thick, sticky paste.
Phase 2: Pradhana Karma
The patient is seated comfortably. The crown of the head (the anatomical equivalent of the Sahasrara Chakra, or vertex) is parted to expose the scalp directly.
The cold, medicated paste is thickly applied directly onto the center of the crown in a circular patch, ensuring maximum contact with the scalp's porous surface.
A soft lotus leaf or a clean cotton cloth is placed over the wet paste, and it is tied securely with a headband. The paste is retained on the head for 30 to 45 minutes.
Phase 3: Pashchatkarma
Once the paste dries slightly or the retention time is complete, the cloth is untied and the thick paste is gently scraped off the scalp.
Because the paste induces massive cooling, a tiny pinch of Rasnasnadi Choorna (a warming powder) is instantly rubbed into the exact spot to prevent a cold or sinus blockage.
The patient wipes their head clean. They are strictly advised to rest in a quiet, dimly lit room, avoiding loud conversations, bright screens, and immediate hair washing.
